1. Contact our office to schedule an appointment with one of our Care Consultants. We will talk with you and ask you a few brief questions over the phone to determine what services you may need.
2. Our consultant will meet you at your location, whether it is at home, in a skilled nursing, rehab or in the hospital. We will discuss what changes are going on in your life and determine what services and/or community will meet your needs. They will then assess what you are able to do independently and will be able determine if you are able to meet the requirements for an Independent, Assisted Living or Memory Care Community. In the event that you do not meet the requirements, we will take the time to discuss further options.

We will discuss your finances to help fit you with the appropriate community that fits your budget. We will also discuss financial resources that can save you money should you qualify.

We will talk about the area you were most interested in living and activities you most enjoy. Our purpose is to find the community that best fits your physical as well as your social needs.

1. We will then schedule tours at 3 – 4 communities, providing transportation should you need it. Following the tours we will discuss all the pros and cons with you, assisting you in the decision process.

After you have had a chance to discuss the options we have shown you we can help with the medical paperwork to help facilitate your move.
